Two days of vomiting in this heat empties you faster than most people expect. You stand up and the room tilts, your mouth is dry, and every sip of water comes straight back. At that point drinking is not the solution, because nothing is staying down long enough to be absorbed. How to tell you are genuinely dehydrated
Thirst is unreliable in the tropics because you are sweating and drinking constantly. The signals worth trusting are:

What is actually in the drip
The base is a sterile balanced salt solution, and that is what genuinely corrects dehydration. Everything else is an addition chosen for your specific problem — anti-nausea medication so you can hold water down afterwards, something for pain or fever, an antacid where the stomach lining is inflamed, and B and C vitamins where useful.

Why people need one here
The usual causes are food poisoning, traveller’s diarrhoea, heat exhaustion and viral fevers including dengue.
Railay adds two of its own. The viewpoint and lagoon trail is a steep mud-and-rope scramble that people attempt in the middle of the day with a single small bottle of water, and it produces heat casualties reliably. And climbers dehydrate quietly — a full day at the crag in humidity, chalk drying the hands and mouth, with drinking treated as an afterthought between routes.

How long it takes and what it feels like
A standard bag runs over about forty-five minutes to an hour, and most people are in and out inside ninety minutes including the consultation. You sit in a reclining chair with air conditioning and can use your phone throughout.
The cannula going in is a short scratch and then nothing. Stinging or a cold ache travelling up the arm usually just means the rate needs adjusting — say so and it is easily fixed.
If you are badly dehydrated the nurse may run two bags. You will usually be asked to pass urine before leaving, because that is the honest test of whether it worked rather than simply whether you feel better.
When a drip is the wrong answer
A drip treats dehydration. It does not treat the cause, and there are situations where reaching for one first is a mistake.
Blood in the stool, a fever above 39C that will not settle, severe pain concentrated in one spot, vomiting beyond two days, or any confusion need a diagnosis before they need fluid. Severe pain fixed in the lower right abdomen is appendicitis until proven otherwise — and on Railay that is a same-day crossing to the mainland, not something to sleep on.
You will always be examined first. If something else is going on you will hear about it rather than be quietly sold a bag of saline.
Getting a drip without leaving your room
Most people who need fluids least want to walk for them, which is the argument for a doctor resort visit. A nurse can set up and run the full drip in your room across Railay West, Railay East, Phra Nang and Tonsai, staying for the duration rather than leaving it running.
Afterwards, keep sipping oral rehydration solution rather than plain water for the rest of the day — a drip corrects the deficit but does not stop ongoing losses if the diarrhoea has not settled. Eat plain food when you feel able.
If you are no better by the next morning, come back rather than booking a second drip on the assumption that more fluid is the answer. Persistent vomiting on a peninsula with no hospital is a reason to reassess, not to repeat.

You can ask, and you will be examined first. If you are well and hydrated you will be told honestly that the evidence for routine vitamin infusions is thin, rather than sold one.
