Nearly every claim needs the same four things:
- An itemised receipt showing each charge separately, not a single total
- A medical report giving the diagnosis and the treatment given
- Proof of payment
- The dates of treatment

Tell us before the visit, not after
Mention insurance at reception when you arrive, not when you are paying.
Some insurers will settle directly with the clinic, which means you pay nothing up front. That has to be arranged during the visit while there is time to make the call — once you have paid it becomes a reimbursement claim and the process is entirely on you.

Why claims get rejected
The reasons are consistent and mostly avoidable:
- Rock climbing, deep water soloing or another excluded activity
- Not notifying the insurer within their window, often 24 to 48 hours
- A pre-existing condition not declared when the policy was bought
- Alcohol involvement, which most policies exclude
- Treatment considered routine or elective rather than an emergency
Keep everything: receipts, reports, prescriptions, and the boat fare, which on Railay is a genuine transport cost and is often reimbursable. Photograph each document as you receive it — paper receipts do not survive a fortnight in a beach bag.
If you need documents after you have left
This happens constantly. You get home, the insurer asks for one more thing, and the clinic is several time zones away.

If your insurer needs a specific form completed or particular wording added, send it over and the nurse who actually treated you will complete it.

If you have no insurance, or a claim is refused
A first refusal is not the end of it and a surprising number are overturned. Ask the insurer for the specific policy clause in writing — refusals are often based on an assumption rather than the facts. Once you know the clause, the response is usually a short clarifying letter from the treating doctor addressing that exact point, which we will write.
If you are uninsured, it is worth knowing this is not a catastrophe at clinic level. Private outpatient care in Thailand is inexpensive by European, Australian and North American standards, and a consultation with tests and medication often costs less than the excess on a policy.
Where it genuinely matters is hospital admission, surgery and evacuation, which can run to very large sums. If you are uninsured and something serious happens, say so early — it changes what is worth doing here versus referring, and there is usually a sensible route that does not end in an open-ended hospital bill.
