  • Fever above 38.5C
  • Severe pain concentrated in one spot rather than general cramps
  • Symptoms continuing beyond three days
  • Any of the above in a young child or an older traveller

Severe pain fixed in the lower right abdomen deserves same-day assessment — appendicitis is regularly mistaken for food poisoning in the first day, and on Railay that is a decision about crossing rather than about waiting.
