Viral or bacterial — how to tell
Broadly, a cough and a runny nose point towards a virus, and their absence points towards strep. The features that raise the likelihood of bacterial infection are:
- Fever above 38C
- White patches or pus on the tonsils
- Tender, swollen glands in the neck
- No cough and no runny nose
- Sudden onset rather than a gradual build-up
Even with all of these it is not certain, which is precisely why a swab is worth doing rather than treating on impression.
Why antibiotics are not automatic
Most sore throats are viral and antibiotics do nothing at all for them. They will not shorten the illness, they carry side effects, and overuse drives resistance — a particular problem in Thailand, where antibiotics have long been available over the counter.
A rapid test gives an answer in about ten minutes. If positive, antibiotics genuinely help and prevent the rare complications of untreated strep. If negative, we treat the symptoms properly, which for a bad viral throat still makes a substantial difference to the next three days.
Why throats get sore here
Air conditioning is the biggest single cause and the one nobody suspects. A bungalow held at 18C all night dries the throat lining thoroughly, and people wake with a raw throat that has no infection behind it. Set it to 24 or 25 and point the airflow away from the bed.
Add dehydration, a long-haul flight on arrival, shouting over music at a beach bar, and — specific to here — a day of breathing chalk dust under an overhang, and it is entirely possible to arrive at a genuinely painful throat with no virus involved at all.
What actually relieves it
Simple measures work better than people expect. Paracetamol or ibuprofen for pain, taken regularly for the first couple of days rather than only when it becomes unbearable. Gargling with warm salt water several times daily. Anaesthetic lozenges or throat spray. Cold drinks and ice.
Honey in warm water soothes and has reasonable evidence behind it. Turn the air conditioning up. And if your nose is blocked and you have been mouth-breathing all night, treating the sinus congestion often resolves the throat as a side effect.
When a sore throat is urgent
Get to a hospital immediately if there is difficulty breathing, difficulty swallowing your own saliva, drooling, a muffled hot-potato voice, severe pain on one side only with the jaw difficult to open, or swelling of the neck.
Those suggest a quinsy or a deeper neck infection, which need drainage rather than tablets. Pain confined to one side that is worsening after several days of antibiotics is the classic pattern, and it needs seeing the same day.

How long it should take to settle
A viral sore throat peaks at two to three days and settles within about a week. Strep throat improves noticeably within 24 to 48 hours of starting antibiotics, and you stop being contagious around the same point.
Come back if pain is still worsening after five days, if it becomes confined to one side, if you cannot swallow fluids, or if the fever returns after settling.
A sore throat that drags on for weeks without fever is usually reflux, allergy or persistent air-conditioning dryness rather than infection, and repeated antibiotics achieve nothing.
