The waiting is the worst part. Whether it was a condom that failed, a night you cannot fully piece together, or simply a routine check you have been putting off for years, the uncertainty weighs more than the test ever does. How the rapid test works
A small blood sample is taken from a fingerprick or a vein. The test looks for antibodies your body produces against HIV and, separately, for the p24 antigen from the virus itself, which appears earlier than antibodies do. That combination is a fourth-generation test and it detects infection sooner than older antibody-only tests.
You wait at the clinic and the nurse gives you the result in person. Nobody is handed a positive result by message or left to interpret a printout alone.
When the test becomes reliable
This is the question that matters most and the one answered worst online.
A fourth-generation test detects the large majority of infections from about four weeks after exposure, and a negative at twelve weeks is conclusive. A test taken three days after a risky night tells you about your status three months ago — it says nothing about that night.

If the exposure was in the last 72 hours
Testing is not the priority in that window — prevention is.
PEP is a 28-day course of anti-HIV medication that can stop an infection establishing itself, and it must begin within 72 hours of the exposure. It works better the earlier it starts, and the difference between six hours and sixty is meaningful.
Come in straight away and ask about PEP and PrEP. Do not wait for a test result — a baseline test is taken at the same time as starting the medication, not before deciding on it. If it is outside clinic hours, message us.
What a positive result actually means now
Worth stating plainly, because many people carry fears formed from what they absorbed decades ago.
HIV today is a manageable long-term condition treated with tablets, usually one a day. People diagnosed early and on effective treatment have a normal life expectancy. They also cannot transmit the virus sexually once it is undetectable in the blood — undetectable equals untransmittable, which is settled science rather than reassurance.
A positive result is a serious diagnosis that needs treatment and follow-up, and it is not what it was in the 1990s. If your result is positive we explain the next steps, arrange confirmatory testing, and refer you either to a Thai hospital HIV service or give you documentation to take home.

Testing is confidential. Results are given to you and recorded in your own file. Nothing goes to your resort, employer, insurer or embassy, and there is no reporting of results to any government.
Railay in season is a small place where a lot of people know each other, so it is worth saying explicitly: nobody at the clinic discusses who came in or why, and you may use a first name only. A passport is not required to be tested.
If you are also worried about other infections, a broader STD testing panel can be run from the same blood draw rather than needing a second appointment.
Testing as routine rather than crisis
Most people come in after a specific worry, but there is a good argument for testing periodically regardless — particularly for the seasonal population who live here for months at a time.
A test every three to six months turns a frightening one-off event into an unremarkable piece of admin, and it catches infection at the stage where treatment works best and transmission risk can be brought to zero. It also removes the awful arithmetic of trying to work out which encounter to worry about.
Worth knowing too: sharing needles is a risk, but a tattoo or piercing from a shop using new sealed single-use needles is not, and neither are mosquito bites, shared food, pools or toilet seats. If you are anxious about an exposure that falls outside the genuine categories, come and describe it — five minutes to be told there is nothing to test for is a far better outcome than months of quiet worry.

How much does an HIV test cost?
A rapid HIV test is among the cheaper tests we run. Adding syphilis, hepatitis or a full panel increases the cost, and you are quoted before any blood is taken.
Is the test anonymous?
It is confidential, and you can use a first name only. Results are released to you and nobody else.
Can I be tested at my resort?

What if I am too early to test?
Come anyway. We take a baseline, explain when a repeat becomes conclusive, and if you are inside 72 hours of the exposure we can discuss preventive treatment at the same visit.
Does a negative test mean I am clear of everything?
No — it means HIV was not detected within the limits of the window period. Chlamydia, gonorrhoea, syphilis and hepatitis all need their own tests, which is why a panel is often more useful than a single test.
