What to bring

Why a nurse has to see you
Thai pharmacies sell a great deal over the counter, but ongoing medication needs an assessment, and it is not simply a formality to justify a fee.
Blood pressure medication needs your blood pressure checked, because a dose that suited you in a cold country is often wrong after two weeks of heat and sweating. Diabetes medication needs a glucose reading for the same reason — appetite, activity and alcohol all change on a trip like this. Both checks take two minutes and both occasionally reveal that the dose now needs adjusting.
It also avoids interactions with anything you have been given here for an injury or an infection.
What we can and cannot replace
Most everyday long-term medications are straightforward: blood pressure tablets, statins, diabetes medication, thyroid replacement, asthma inhalers, contraceptives, antidepressants, acid reflux medication and antihistamines.
Strong opioid painkillers, sleeping tablets and stimulants such as ADHD medication are tightly controlled in Thailand. Some cannot be prescribed by a clinic at all, and a few are illegal to bring into the country even with a prescription from home — which occasionally becomes a serious problem at the border rather than at a pharmacy.
If you take any of these, say so early so you get a straight answer rather than being sent round three pharmacies to find out the hard way.
If the exact brand is not available
Thailand stocks most drug classes but not every brand. Usually there is an equivalent with the same active ingredient at the same strength, and the nurse will explain the substitution and write down both names so you can match it up at home.
Occasionally the direct equivalent is unavailable and the nearest alternative is in the same class but not identical. That is a genuine change to your treatment rather than an administrative detail, so it gets discussed properly — including what to watch for over the following days.
Availability is also affected by being on a peninsula. Some items need ordering from the mainland, which adds a day. Ask early rather than on the morning of your last packet.
Contraception, and running out mid-trip
The contraceptive pill is one of the commonest requests. Bring the packet if you possibly can, because there are many formulations and matching it properly matters more than with most medications.
If you have already missed pills because the packet ran out, say so plainly and say how many, and when. That changes the advice about whether extra precautions are needed and for how long, and it may make emergency contraception relevant — which is available from Thai pharmacies without a prescription.
If there was also unprotected sex with a new partner, it is worth considering STD testing at the same visit rather than making a second trip for a conversation you have already started.
Planning ahead for a season
If you are here for months rather than weeks, convert your supply into something sustainable rather than repeatedly running out at the worst moment.
Bring a copy of your repeat prescription and a summary from your own doctor listing conditions, medications and doses. With that, a longer supply can usually be arranged and generic equivalents identified — and Thai generics are frequently far cheaper than the equivalent at home, which surprises most people.
A health check up is worth combining with this if you have been on the same medication for years without monitoring. Blood pressure tablets, statins, thyroid replacement and diabetes medication all warrant periodic bloods, and doing it here is quick and inexpensive.

How much can I be given at once?
Enough to cover the rest of your stay in most cases. For controlled or long-term medication the nurse may give a shorter supply and ask you to return, which is about safety rather than inconvenience.
