How a nebuliser differs from your inhaler

What happens during treatment
Your oxygen saturation, pulse and breathing rate are measured first, and the nurse listens to your chest before anything is given. The mask goes on and you breathe normally for about ten minutes while the medication runs through.
You are reassessed afterwards, which matters as much as the treatment. Most people improve markedly within that first session. If the response is only partial a second dose may follow, sometimes with a steroid to settle the underlying inflammation.
Expect a slightly fast heartbeat and mild shakiness afterwards — that is the medication working as expected rather than a bad reaction, and it passes within the hour.
When breathlessness means crossing to the mainland

- Drowsiness or confusion
- A silent chest — wheeze that stops while breathing gets harder
- No improvement after a nebuliser, or symptoms returning within an hour

Managing asthma on a climbing trip
A flare-up nearly always has an identifiable trigger. Air conditioning set very cold is the commonest, followed by a dusty bungalow, chalk, and smoke from beach fires or burning.
Keep a reliever inhaler physically with you rather than in the room — on this peninsula, “back at the bungalow” can be a twenty-minute walk over rock, which is the wrong distance in a flare. Do not stop your preventer inhaler because you feel well; most flare-ups follow a few skipped days.
If you dive, an uncontrolled flare means diving is off until it has fully settled, regardless of what any certificate says. That is a safety limit rather than paperwork.
After the treatment
A nebuliser opens the airway; it does not treat whatever closed it. What happens next depends on the cause, and leaving without that conversation is how people end up back the following evening.
If the trigger was an asthma flare, the preventer usually needs restarting or stepping up, and a short course of oral steroids is often the difference between settling and relapsing. If the cause was a chest infection, that needs treating in its own right.
You will be given a clear plan and a threshold for coming back: symptoms returning within four hours, needing the reliever more often than every four hours, or any of the warning signs above.
