Rabies Vaccination in Railay
The macaques at Phra Nang are the most photographed animals in Krabi and the most common cause of rabies exposure on this peninsula. They are bold, they are fast, and they will take food from a hand. If the skin was broken, this is genuinely urgent — rabies is effectively always fatal once symptoms appear, and almost entirely preventable if treatment starts in time. Rabies vaccination in Railay can begin the same day, on the peninsula.
Wash the wound first — this matters more than anything
Before you walk anywhere, wash the bite or scratch under running water with soap for a full fifteen minutes.
Fifteen minutes feels absurd standing at a tap. It is the single most effective step in the entire process and measurably reduces risk before any needle is involved. No amount of later treatment substitutes for it.
Do not scrub raw tissue with strong antiseptic, and do not close the wound with a plaster or tape. Wash it thoroughly, cover it loosely, and come in.
The Phra Nang monkeys specifically
Almost every monkey bite we treat follows the same sequence: someone is carrying visible food or a drink, a macaque approaches, they try to hold on to the bag, and the animal takes a hand.
The practical rules are simple and widely ignored. Do not carry food or drinks openly on the Phra Nang path or beach. Do not hold a bag loosely. Never try to take something back once a monkey has it — a plastic bag is not worth a course of rabies injections. Do not feed them under any circumstances, and do not let children hold food near them, because children are bitten on the face and hands far more often than adults.
Macaques also scratch more often than they bite, and a scratch that barely bled still counts as an exposure if the skin was broken.
What counts as exposure
More than people assume. Any bite or scratch from a monkey, dog, cat or other mammal counts if the skin was broken. So does a lick on skin that was already broken.
The animal appearing perfectly healthy proves nothing — an infectious animal can look and behave normally for days before it sickens, and you will never see that macaque again to check. Treatment therefore proceeds on the assumption that exposure is possible, which is the standard approach worldwide.
Thailand records human rabies cases most years. This is not a theoretical risk being managed defensively.
What the course involves
If you have never been vaccinated against rabies, you need a series of injections into the arm over roughly a month — typically day 0, 3, 7 and a final dose between day 14 and 28.
For deeper or higher-risk wounds, rabies immunoglobulin is also infiltrated around the wound at the start, giving immediate protection while your own immunity builds. It is the expensive part and the part that is not always easy to source at short notice, which is worth knowing on a peninsula reached only by boat.
If you were vaccinated before travelling, you need only two booster doses and no immunoglobulin at all — which is the strongest practical argument for pre-travel rabies vaccination if you are here for a season.
Finishing the course after you leave
Most travellers leave part-way through the schedule and that is entirely workable, provided you finish it.
We give you a written record with the vaccine name, manufacturer, batch number and the exact dates each remaining dose is due, so any clinic in any country can continue without restarting. Photograph it as well as keeping the paper.
Do not abandon the course because you feel completely fine. There are no symptoms during the incubation period, which can run weeks or months, and by the time there are symptoms it is far too late for treatment to do anything.
The other risk from a bite
Rabies takes the attention, and ordinary bacterial infection is what actually sends most people back. Monkey bites are dirty, and hand bites in particular seal over quickly and trap bacteria in the tissue underneath.

Frequently asked questions
How soon do I need the first injection?
The same day if at all possible. There is no cut-off after which it becomes pointless, but the sooner it begins the better it works — and on Railay, sooner also means before the boats stop.
How much does the rabies course cost?
Each dose is priced individually, and immunoglobulin where needed costs considerably more than the vaccine. You are told the full cost of the course at the first visit.
The monkey looked healthy. Do I still need it?
Yes. An animal can transmit rabies before it appears unwell, and you will never see that macaque again to check. The treatment is safe and the disease is not survivable.
Can I get the injections at my resort?
Follow-up doses can be given on a doctor resort visit. The first visit is better at the clinic so the wound can be irrigated properly.
I had the pre-travel vaccine. Does that cover me?
It changes the treatment rather than removing it. You still need two booster doses, but not immunoglobulin — which is the expensive and harder-to-source part.
Should I get vaccinated before coming to Railay?
